Family Practice Associates is now offering patients the opportunity to access their health records online. This will occur through a secure, HIPPA compliant, on line portal. When you come in for your next visit you will be asked for your email address and it will be intered into our Electronic Medical Record. When your lab results come in you receive an email from your doctor notifiying you of your results. You will also be able to access some parts of your medical record that you can provide to your other doctors.
You will receive an email “Welcoming you to the Family Practice Secure Patient Portal.” It is highly recommend saving this email in order to access this in the future.
You then will receive another email giving you a Username and Password to access the “Patient Portal,” it’s highly recommended to write down the Username and Password. On the “Welcoming you to the Family Practice Secure Patient Portal” page you will then click on “Click Here To Sign in” You then will type in Your Username and Password
Verify your Account. At this time it will ask for your date of birth.
Once these steps are done, you will then be able to access your records by simply clicking on the tab that says “Records” that is located on the left of the page. Any Messages that are sent from the doctor or the doctor’s nurse can be reviewed by clicking the tab that says “Messages” located on the left side of the page as well. To access your Records, click on the “Records” tab that is located on the left side of the page. You will choose to either review your labs as a data file (in XML format), which you can download and provide to other practices or upload to a personal heath record (such as Microsoft Health Vault) as a human readable report (in HTML or PDF format), which you can read yourself.
IF YOU DO NOT GET YOUR RESULTS WITHIN 5 BUSINESS DAYS OF YOUR OFFICE VISIT PLEASE CHECK YOUR JUNK MAIL! It is possible that the results make have been identified as junk and put in your junk email box and not your inbox.
