What is an ACO? ACOs are groups of doctors, hospitals and other health care providers, who come together voluntarily to give coordinated high quality care to their Medicare patients.
Our goal at Family Practice Associates of Chesterfield is coordinated care utilizing available resources to ensure that our patients receive all the benefits of Medicare offers while still having dedicated and compassionate one on one care with the provider.
Medicare Wellness Exams
A Medicare wellness visit is an annual exam with a primary care doctor for people on Medicare. The purpose of this visit is to create or update a personalized plan to help prevent illness based on risk factors and the current state of health. It is not the same as the annual physical you received from your insurance before you were eligible for Medicare. Medicare does not cover annual physicals as a benefit.

Transition of Care
Transitional care management (TCM) is intended to reduce potentially preventable readmissions and medical errors during the 30 days following discharge from the acute care setting such as a hospital or rehab/nursing home facility
Chronic Care Management
Chronic care management includes a comprehensive care plan that lists your health problems and goals, other providers, medications, community services you have and need, and other information about your health. Depending on your personal health history, you are interested in adding this service to the care provided by Family Practice Associates of Chesterfield.
